LightMedSeg-ISLES: Stroke Lesion Segmentation with 81x Fewer Parameters than nnU-Net

Pre- and Post-Treatment Brain Metastases Segmentation Using nnU-Net with Post-Processing for BraTS 2026

The paper presents a segmentation pipeline for brain metastases in both pre‑ and post‑treatment cases using a 5‑fold nnU‑Net ResEnc‑L ensemble trained on 1,296 four‑modality cases. A rule‑based post‑processing cascade improves the lesion‑wise Dice similarity coefficient (LW‑DSC) for enhancing tumour, tumour core, whole tumour, and resection cavity sub‑regions, achieving LW‑DSC scores of 0.733, 0.751, 0.713, and 0.549 respectively on the official validation leaderboard. The authors conduct a five‑fold out‑of‑fold analysis to validate the robustness of each post‑processing stage, provide a mechanistic explanation of LW‑DSC behaviour, and report thirteen negative results that challenge common intuitions, with all code released under Apache‑2.0.

Exemplar: Classical Priors Complement Frozen Features for Few-Shot Microscopy Segmentation at Native Resolution

Exemplar is a few‑shot segmentation method that combines a frozen DINOv3 backbone with a fixed bank of classical native‑resolution filter responses in a single lightweight head. Trained only from support masks, it achieves a mean foreground intersection‑over‑union of 0.782 across eleven biomedical imaging datasets, outperforming either component alone and surpassing five other few‑shot methods in 54 of 55 comparisons. With a single annotated mask, Exemplar reaches 0.703, higher than a from‑scratch nnU‑Net trained on the same mask, and while nnU‑Net eventually overtakes it with eight masks, it requires 16–77× longer to fit.

Spectral Adapters for Segment Anything Model-based Segmentation of Colorectal Liver Metastases in Computed Tomography

The paper introduces two lightweight spectral adapters—Directional Spectral Adapter (DiSECT) and Spectral Instance-Guided Adapter (SiGA)—to adapt the Segment Anything Model (SAM) for accurate segmentation of colorectal liver metastases in contrast‑enhanced CT scans. SiGA achieves the highest single‑point Dice score of 0.77 and performs comparably to a 3D nnU‑Net baseline under no‑prompt inference, while DiSECT requires only 0.14 million trainable parameters. The study evaluates the adapters on 446 CT volumes across various prompting regimes, demonstrating that spectral adapters can efficiently adapt SAM with limited trainable parameters while maintaining strong segmentation accuracy.
