WILSON - a pathology foundation model framework for patient-level analysis and diagnostic text generation

WILSON is a vision–language foundation model that represents whole‑slide images and multi‑slide patient cases as single multi‑magnification composite images. Trained on about 189,000 Mayo Clinic slides covering 42 organs and 829 diagnostic entities, it outperforms dedicated case‑level models on internal cohorts and matches slide‑level models while using far less compute. Fine‑tuning on triple‑negative breast cancer data improves histologic subtyping and lymphocyte grading, and the model retrieves diagnostic text with high recall and generates captions closer to report references than prior methods.

Beyond Sparse Captions: Aligning Slide-Level Text and Patch-Level Vision in Pathology

The paper introduces Locked Image Multiple Instance PreTraining (LIMIT), a method that aligns slide‑level pathology reports with patch‑level visual features by freezing a vision encoder and fine‑tuning a text encoder. LIMIT uses report embeddings as cross‑attention queries over bags of patch features, optimizing slide representations via InfoNCE. Applying LIMIT, the authors create CONCH‑Z, which achieves state‑of‑the‑art zero‑shot performance on 22 pathology tasks, outperforming vision‑language encoders trained on curated caption datasets and narrowing the gap with larger slide‑level foundation models.

PathAgentBench: Benchmarking Evidence-Seeking Vision-Language Models on Whole-Slide Pathology Image

Whole-slide image (WSI) diagnosis requires identifying diagnostically relevant regions, examining them across magnifications, and integrating multi-scale evidence. However, most existing pathology benchmarks evaluate models on pre-cropped patches or pre-extracted slide features, leaving their ability to acquire evidence directly from gigapixel WSIs largely untested.

Morphology signal in whole slide image foundation models can automatically triage slides

The paper introduces a pipeline that uses publicly available whole slide image foundation models (FMs) to automatically triage slides by ranking them based on zero‑shot classification predictions. This approach accurately identifies slides containing the most tumor, achieving top‑2 ranking for patients with up to 43 slides across multiple datasets. The study also proposes a ranked evaluation framework to benchmark FM performance in slide triage.

SlideBank: A Persistent Hierarchical Evidence Bank for Consistent Whole-Slide Reasoning

SlideBank is a training‑free framework that turns each whole‑slide image into a persistent, concept‑indexed evidence bank. It performs coarse‑to‑fine exploration to locate informative regions and multi‑scale views, converts them into explicit morphological observations, and anchors pathology signals to the supporting patches and slide coordinates. During inference, questions are routed to relevant signals and evidence scales, and a confidence‑based cross‑level consensus integrates global, regional, and patch evidence, achieving high accuracy on WSI‑VQA and SlideBench‑BCNB while enabling consistent re‑phrasing and reduced inference cost.

How Ground Truth Shapes Retrieval Scores: Evidence-Aware Evaluation of a Retrieval-Augmented Generation System for Breast Lesions

The study introduces an evidence-aware evaluation protocol for a retrieval‑augmented generation system focused on breast lesion management literature. Using an 18‑document corpus and 18 queries, the authors compared dense embeddings, BM25, and cross‑encoder reranking across various chunk sizes and overlaps, finding that a hybrid retriever with 384‑token chunks and 20% overlap achieved top‑three retrieval success for 17 of 18 queries and high recall and precision metrics. Expanding ground‑truth relevance annotations increased recognized evidence spans and accepted chunk targets, though retrieval rankings remained unchanged, underscoring the sensitivity of evaluation to annotation coverage.
