CARE: Confidence-Aware Reasoning for Reliable Medical VQA

arXiv:2608. 10964v1 Announce Type: cross Abstract: Reinforcement Fine-Tuning (RFT) has enabled medical Multimodal Large Language Models (MLLMs) to produce Chain-of-Thought (CoT) reasoning for visual question answering, yet these models suffer from $\textit{confidence miscalibration}$---a systematic gap between expressed certainty and actual diagnostic accuracy that undermines clinical trust.

Deterministic Hallucination Detection in Medical VQA via Confidence-Evidence Bayesian Gain

arXiv:2603. 21693v2 Announce Type: replace Abstract: Multimodal large language models (MLLMs) have shown strong potential for medical Visual Question Answering (VQA), yet they remain prone to hallucinations, defined as generating responses that contradict the input image, posing serious risks in clinical settings.

When Seeing Overrides Knowing: Visual Dominance and Deferral-Based Method for Personalized Safety in VLMs

The paper introduces MPS-Bench, a benchmark of 5,181 scenarios from 584 real-world images across 12 high-risk domains, each paired with a hidden user profile, to evaluate personalized safety in vision‑language models (VLMs). Eight leading VLMs were tested and found to almost always respond directly (86‑99%) without seeking missing context, scoring no higher than 2.6/5 on personalized safety. The authors identify a phenomenon called visual dominance, where visual information enters text representations early and suppresses textual risk signals, and propose PRISM, a lightweight input monitor that predicts when a query should be deferred, achieving 0.978 AUC and outperforming all tested models on the safety‑utility Pareto frontier.

NV-Reason-CT: 3D Visual Language Model for CT Analysis

NV-Reason-CT is a generative vision‑language model designed for chest and abdominal CT analysis that preserves native 3D visual encoding and incorporates radiologist‑guided reasoning. The system couples a 3D vision transformer with a language model, feeding all visual tokens and their 3D coordinates directly into language decoding to maintain volumetric spatial information. Trained on a curated corpus of about 550,000 multimodal instruction examples, the model supports abnormality classification, report generation, and interactive reasoning, achieving strong performance on CT benchmarks and reducing expert interpretation time by 50%.
